The role of the nurse in general began as early as the 1800s, where nurses mainly performed simple tasks such as bathing and feeding patients. The role of the nurse began to change in the late 1800s during war, where nurses were trained out in the field by the physicians, to better assist and take on more responsibilities in complex medical situations due to a small physician population. Even still, nurses were not valued for the roles that they performed. It was around the 1950s and 60s when the actual title of Advanced Practice Nurse was given to those nurses that were educated and trained at a higher level to perform more complex tasks such as assessment and diagnoses, anesthesia administration, and nurse midwives. Nurses started to be recognized for having advanced education and training in certain areas and were being considered experts in their specialty.

When nurse training started to become more formalized, training was provided by the institution that the nurse worked for. This created a problem within the nurse training because training from the different hospitals varied due to differences in policies and the way treatment was rendered. Even though the number of nurses increased, there was still an issue with the delivery of care. It was in the 20th century when nurse training started to evolve and become more uniformed. The education of the nurse was transitioned to colleges and taken out of the hospital. Throughout the 20th and 21st centuries the training of nurses continued to evolve. As the training improved and became more focused, the roles of the nurse changed and evolved as well.

The International Council of Nurses (ICN) defines Advanced Practice Nurse as, “A Nurse Practitioner/Advanced Practice Nurse is a registered nurse who has acquired the expert knowledge base, complex decision-making skills and clinical competencies for expanded practice, the characteristics of which are shaped by the context and/or country in which s/he is credentialed to practice. A master’s degree is recommended for entry level.”(ICN, 2013) As the healthcare system becomes more complex, the roles and responsibilities of the advanced practice nurse will continue to increase and transition to more areas of care such as research, policy development and intervention, and primary care provider. Advanced practice Nurses of today work alongside Physicians to improve the quality of care provided.

According to DeNisco and Baker (2021), nursing education has transitioned from an apprentice style of education to one that involves an education built in liberal arts requiring the development of critical thinking abilities. Even the term advanced nursing practice has been shrouded in considerable uncertainty until a watershed seems to have been reached for a definition that indicates master’s-prepared nurses who provide direct clinical care and to include the roles of nurse practitioner (NP), certified nurse–midwife (CNM), certified registered nurse anesthetist (CRNA), and clinical nurse specialist (CNS) (DeNisco & Baker, 2021). The 2010 report by the Institute of Medicine (IOM) that identified the need for nurses to be positioned as vanguards of health care and strongly advocating for allowing advanced practice registered nurses to practice to the full scope of their skills and the removal of barriers to enable forward movement of nurses and the nursing profession paved the way for the advancement of nursing (Stewart, 2021).

The nurse practitioner practice is thought to have emerged in the 1960s taking cue from Lilian Wald’s nurses of the 1800s and was developed to provide primary care underserved population (Stewart, 2021). In 1965, Loretta Ford and Henry Silver formally developed the role of nurse practitioner to advance the nursing profession and to respond to the need for providers in rural, underserved areas (Stewart, 2021). DeNisco and Baker (2021) affirmed that the role evolved out of the unavailability of primary care physicians to take care of pediatric populations with educational preparation changing from the initial 3 to 12 months to the present-day master’s degree as the entry-level education for nurse practitioner specialties.

Formal midwifery training was also necessitated to solve an urgent need to curb high maternal and infant mortality rates which led to the establishment of the first nurse–midwifery school in 1925 by Mary Breckenridge. Today, all nurse–midwifery trainings are college and university level programs which culminate in the DNP degree. The practice of midwifery requires licensed and independent certified nurse–midwives and certified midwives (CMs) who are autonomous primary care providers with prescriptive authority to manage women’s health, that concentrate on pregnancy, childbirth, the postpartum period, care of the newborn, family planning, and gynecologic needs of women (DeNisco & Baker, 2021).

Having evolved more than 150 years ago, nurse anesthetists are another licensed independent practitioners who autonomously deliver care in collaboration with surgeons, dentists, podiatrists, anesthesiologists and other healthcare professionals (DeNisco & Baker, 2021).

DeNisco and Baker (2021) averred that there is a national initiative to improve access to high-quality but cos-effective health care. This directive is bound to necessitate the advent of many new and yet to be conceived roles for nurses. Newly identified nursing roles such as nurse navigators and healthcare coaches, have been instituted and may require advanced degrees and certification. There are thriving care coordination opportunities for nurses throughout the continuum of care (DeNisco & Baker, 2021).

I agree with the above statements. I was detailed back to my old unit in dialysis when there were no surgical procedures at the beginning of the pandemic and I was willing to help out with the demands of healthcare during those times. I was very competent to render care because of my past skills. The problem was the other nurses who were detailed on other floors where they had no past experience were all thrown out without educational support and it built a lot of mental anxiety on top of physical exhaustion. After the first and second surge of the pandemic, several nurses opted for early retirement and some ventured to learning other skills that allowed them to work at home because of the fear of uncertainty and dismay from planned strategies of their management and leadership of the organization/state/health department.

The role of advanced registered nurse has undergone significant transformation over time. The transformation can be seen in the changes in roles between the 20th and 21st century. The transformation in roles is largely attributed to the influence of factors that include professional, government, legal, and technological changes. The advanced registered nurse did not rely mainly on health technologies in the 20th century.

This changed in the 21st century where novel healthcare technologies are largely utilized to provide high quality, safe and efficient care to diverse populations. The 20th century nurse also provided fee-for-service care. The focus of nursing care was largely on the quantity of care given to the patients. Patients also incurred significant expenses in seeking the care that they needed. The focus of nursing care in the 21st century is however different from the above. Advanced registered nurses in the 21st century provide value-based care. The focus of nursing care is on the realization of quality and patient-centered outcomes (Gullatte, 2018). The outcome of this focus include patient-centeredness in case, cost-efficiency and safety in care given to patients.

The roles and responsibilities of the advanced registered nurses will continue to evolve and emerge as the American healthcare system changes in the future. For instance, advanced registered nurses will have to utilize information from the big data to make informed decisions on addressing the current and future needs of the patients. There is also the anticipated changes in the dynamics of the patient population in the future of America. As a result, nurses must be trained to be highly adaptive and responsive to the emerging needs of care for their populations. Technological and legal changes in America will also have a positive impact on the future roles and responsibilities of the advanced registered nurses (Young et al., 2017). Advanced registered nurses will have to ensure that the care that they give is evidence-based, innovative, efficient, and patient-centered to address the emerging needs of care for the diverse populations.

The role of registered nursing has come a long way since the 19th century when Florence Nightingale revolutionized modern nursing on the front lines of the Crimean war and Lillian Wald’s nurses elevated nursing to provide primary medical care for inner city low income families. Initially education of nurses who provided advanced specialized care was in the form of apprenticeship training, but it is now a requirement to have obtained a master’s degree or higher to qualify as an official Advanced practice Nurse. Currently there are four degrees that meet  the criteria to qualify as an advanced practice nurse (APRN). They are Nurse Midwife, Certified Nurse Practitioner, Certified Registered Nurse Anesthetist, and Clinical Nurse Specialist (DeNisco & Barker, 2019). As the demands and expectations of the healthcare system continue to increase, the number of roles qualifying to be an advanced practice nurse are bound to expand. Two roles that have potential to be included as APRNs are the roles of case managers, and nurse navigators. Along with the expansion of the field of APRNs in the near future, there will also be an increase in the expectations of their education, training and responsibilities. 

One example of how the education and training of an Advanced Practice Nurse has changed over time is in the role of nurse practitioner(NP). From the time that the role of nurse practitioner was formally developed in 1965 by Loretta Ford, EdD and Henry Silver, MD, until the 1980s and 1990s, when it moved to the university setting as a master’s level program, most NP training programs were continuing education programs. (DeNisco & Barker, 2019). The goal of the American Association of Colleges of Nursing was to have all entry level Nurse Practitioners be educated at the Doctorate level by 2015, but this has proven “impossible to achieve” (DeNisco & Barker, 2019, p. 30).

Doctorate-level nursing education has become the bar for the role of nurse practitioner because NPs have been proven to be a cost effective way to provide high quality care, and the expectations of nurses in this increasingly complex healthcare system are becoming more demanding. The Institute of Medicine (IOM) has “identified the need for nurses to be placed at the forefront of healthcare” (DeNisco & Barker, 2019, p. 24). Nurse practitioners are currently working alongside other doctorally prepared clinicians such as physicians (MD), pharmacists (PharmD), and physical therapists (DPT) to name a few (DeNisco & Barker, 2019). Requiring nurse practitioners to be doctorally prepared would ideally elevate them to be equal with other doctorally prepared clinicians in their respective fields.

I agree that nurses can push for SDOH in the clinical setting by advocating for changes in policies and procedures to benefit the community for health inequalities. The nurses can research electronic health records and bring them to the organization where they practice to improve the documentation and help the healthcare team follow data that could follow health disparities (Scowart, 2023). Encourage the healthcare team to talk with their patients to determine their views on their health disparities. Nurses can research the greatest health disparities and provide education and community health fairs to benefit people of the community. Nurses can go and advocate to stakeholders to provide financial and support to help bring attention to some health qualities. Nurses within the healthcare team can research and ask questions about the community’s patients and involve other healthcare teams such as social workers, dietary, and mental health. Nurses need to push for advanced education to do research to help find evidence-based practices to improve health and improve the health of the community. Nurses should attend communities and groups at a higher level such as state or national levels to bring together ideas to promote and share data about health determinants (Scowart, 2023).
