So that process required individual applications to each state. As many know, Nursing applications aren’t the easiest of processes to complete. Each state required the same standard, transcripts, background checks, and fingerprint cards. That’s where the similarities ended. Each state had different fees and renewal dates, and continuing education was wildly different from state to state. I am almost embarrassed to say that Missouri doe not require a single continuing education unit to renew your license. Kentucky I believe required 60 and they were category specific. So it was challenging to keep up with, and keeping it all organized was complex and errors could be made. Some scopes of practice for nurses can vary from state to state. In Kentucky for instance Ems crews could pronounce the time of death prehospital. That was not allowed in any other states I was licensed in. Thankfully, I never had to do it, but I had to be educated on it. Arkansas, Missouri, and Kentucky are now all members of the nurse licensure compact (NLC). So three renewals went to one, keeping up with CEUs in different states, do at different times also went away. Not to mention the cost. To me, this would be a pro for a National Nursing License.

The main concern among critics is that a national compact interferes with the oversight purposes of the state nursing boards regarding vetting licensees, tracking them, and coordinating disciplinary actions(Schlette ,J 2022). For disciplinary actions, Missouri has found the solution to be your multistate status is revoked. You are only allowed to practice in your primary licensee state with whatever restrictions are placed on you by that Board of Nursing. Your multistate status is not restored until you have met the disciplinary requirements of the primary state. NLC is a road map to a National Nursing License. With technology also comes new technological problems. What is the solution for telehealth? If a telehealth nurse is caring for a patient in another state what’s the answer ? Which state is their governing body? The state the nurse is in or the state the patient/client is in? I hope the NLC does become national but there will be some opportunities as the process grows and matures.

Professional accountability as it pertains to nursing is upholding the ethical code that was pledged upon receiving the nursing degree and following those guidelines in patient care. The vow taken at either nursing school graduation or pinning ceremony states, “do no harm to self or others while under your care” (Green, 2018). Nursing have access to many different types of narcotics and have the job of giving these IV narcotics to the patients, ensuring that the patient receives the right dose, the right route, and the right medication. I previously worked in a rehab program and two of the counselors that taught the program were previous registered nurses and abused their powers as nurses and would take the leftover IV pain medication home with them to inject themselves with it, and quickly developed a drug problem which cost them their job and livelihood. This was in the early 90’s when narcotic medication administration and waste was not as closely monitored as it is today, which made it easier for nurses to defer narcotics. Professional accountability is providing safe quality care within the nurse’s scope of practice, maintaining confidentiality, and staying within the guidelines of policies and procedures. The future of health care and health equity is dependent upon nurses in the United States (O. T. F. O. N., 2021).

For a nurse to provide safe and competent care to a patient, the nurse must first take care of themselves before they can take care of others. There is a saying that says, “you can’t pour from an empty cup,” and when I personally feel my cup is empty, I make a point to search my heart for what I need right now to feel complete. Lately that has simply been spending an entire day with my husband and 4 dogs, and not even leaving the house. Having to constantly be surrounded by people and answering questions, always in that constant state of problem solving and people pleasing, even to the rude patients that cuss me out. It’s mentally and physically exhausting and draining. My self-care is not leaving home all day or talking on my phone, I sit outside on my porch with my dogs and enjoy the silence. When I first began my nursing career, I was that crazy nurse that would work 6 days out of the week, but after 2 short years of nursing, I’m completely burnt out and I recognize that and the need for my self-care.

The Bible says, “Above all else, guard your heart for it is the wellspring of life,” (The Bible, Proverbs 4:23). This to me means protect yourself and guard your heart because it is the water of life. Nursing is one of the most difficult jobs a person could have, and unless a person has worked and experienced nursing for themselves, it is hard for them to understand the stress and overwhelming job nursing is, that is why self-care and is vital for a nurse’s mental health and spiritual well-being.
